Copy of a govt issued ID
Copy of Tax clearance certificate for fetes and other major music events.
Copy of permission to use venue that doesn’t belong to applicant.
Copy of COVID19 Compliance Plan
Non-refundable processing fee.(Schools, Churches and other Government Departments are exempted.)
Application to be submitted fifteen (15) days, but not less than three (3) days before date of event.
Applicant's age must be 18 years or above
